  • Eligible families must be within 200% of federal poverty level

  • No cost

Program requirements

  • Photo ID for parent or guardian 

  • Child verification form (birth certificate, shot record, etc.)

  • Income verification (pay stubs, Nutrition Assistance Award letter)

  • Proof of address
