How to Replace a Lost or Stolen Passport
If your passport is lost or stolen, you should fill out a Form DS-11 passport application and submit this form in person to your nearest passport acceptance facility. You are not required to know the passport number or issuance date to apply for a new passport.
You may complete question 18 on the Application for Passport as follows:
- Write the name in which the passport was issued and the approximate date of issue.
- Under disposition, mark the appropriate box.
- If the passport was expired, you may write the word “expired” in the blank next to the “other” box.
If your most recent U.S. passport is still valid, you must also complete a Form DS-64, Statement Regarding Lost Or Stolen Passport. Again, the passport information is not needed. Fill in as much as you are able. All of the other questions on this form must be answered in detail.
Forms DS-11 and DS-64 are available through the Glendale City Clerk’s Office or from the Download Printable Passport Applications page. Submit the DS-64 along with your passport application and all necessary documentation and fees to your nearest passport acceptance facility. |
