How do you replace your passport if it was issued more than 15 years ago? If your passport has expired and was issued more than 15 years ago, you must apply for a new passport at your nearest passport acceptance facility. You should fill out a Form DS-11 [PDF Passport Application] passport application and submit the expired passport along with this form.
You may complete question 18 on the Application for Passport Form DS-11 as follows:
- Write the name in which the passport was issued and the approximate date of issue.
- Under DISPOSITION, mark the appropriate box.
- Write the word EXPIRED in the blank next to the OTHER box.
