Where Can I Apply for a Passport?

You can apply for a passport at the Office of the City Clerk, records center, Glendale City Hall, 5850 W. Glendale Avenue, Suite SB-12, Glendale, AZ 85301. Our telephone number is (623) 930-3260.

Passport acceptance service is available every Wednesday, except legal holidays, from 8 a.m.-3 p.m. We do not take passport photos at this location.

When Do I Have to Apply in Person?

You must always apply in person if you meet any of the following criteria:

  • This is the first time you are applying for a passport, and you are a U.S. citizen.
  • Your passport is lost or stolen.
  • Your passport was issued more than 15 years ago.

For children 13 and under, both parents or legal guardians must appear to execute a passport application.

Where Can I Get a Passport Application and Forms?

Passport forms are available from the Glendale City Clerk’s Office, any regional passport agency, a designated post Office, clerks of court, county/municipal Offices, or travel agencies. Check your local telephone directory for locations. Look for Passport Services under the Department of State listing in the federal government blue pages of the directory. Or call the National Passport Information Center at 1-877-487-2778. You can also call us at (623) 930-3260.

When Should I Apply for a Passport?

Apply several months in advance of your planned departure. If you will need visas from foreign embassies, allow more time. If you know when you’re traveling, we recommend filling in your departure date on the DS-11 and DS-82 forms, as these will be processed in order of date of departure. If you need to leave in a hurry, you may also expedite the process for an additional fee.

What Happens to My Passport Application After I Submit It?

When applying at Glendale City Hall, the day that you apply, your application will be sent to the National Passport Office for processing. Your passport will be issued within 25 business days after receipt of complete applications by the National Passport Office. Your passport will be sent to you by mail at the address you provided on your application.

I’m renewing my passport. Do I get the old one back?

Yes, the old, cancelled passport will be stamped expired and mailed to you with your new passport.  It is a good idea to keep it in a safe place as it is considered proof of your U.S. citizenship.

What Should I Do if My Passport Is Lost or Stolen?

If your passport is lost or stolen and it is a valid passport, report the loss on Form DS-64, when you apply, in person, for your new passport. If you are abroad, report the loss immediately to local police authorities and the nearest U.S. embassy or consulate. Remember to write your current address in the space provided in your passport, so that, if it is found, it can be returned to you. For more information, see How to Replace a Lost or Stolen Passport.

I was born abroad. How do I get a birth certificate?

If you were born in a foreign country to a U.S. citizen parent or parents, and your parent registered your birth at a U.S. embassy or consulate in the form of a Consular Report of Birth (Form FS-240), and you would like evidence of your birth and United States citizenship, please submit a written request which includes the following five items:

  • The subject’s full name at birth, and date and country of birth.
  • Parents’ full names, including mother’s maiden name, and their dates and places of birth and nationality.
  • A daytime telephone number.
  • Signature of the subject or parents. A legal guardian’s signature is acceptable only when accompanied by a certified copy of the guardianship papers or court order of adoption.
  • A $20 fee per document. Please send a check or money order made payable to the Department of State.

Special Requirements for Children Under Age 14 As required by Public Law 106-113 Effective July 2, 2001

Effective July 2, 2001, Minors Under Age 14 MUST:

1. Provide Application Form DS-11

NOTE: Please do NOT sign the DS-11 application form until the Passport Acceptance Agent instructs you to do so.

2. Present Proof of U.S. Citizenship (for minors under age 14)

You will need to submit one of the following:

  • Certified U.S.
  • Previous fully valid U.S. Passport
  • Report of Birth Abroad (Form FS-240)
  • Certification of Birth Abroad (Form DS-1350)
  • Certificate of Citizenship or Naturalization from INS

3. Present Evidence of Child’s Relationship to Parents/Guardians (for minors under age 14)

You will need to submit one of the following:

  • Certified U.S. birth certificate (with parents’ names)
  • Certified Foreign Birth Certificate (with parents’ names and translation, if necessary)
  • Report of Birth Abroad (Form FS-240) (with parents’ names)
  • Certification of Birth Abroad (Form DS-1350) (with parents’ names)
  • Adoption Decree (with adopting parents’ names)
  • Court Order Establishing Custody
  • Court Order Establishing Guardianship.

NOTE: If the parent(s)’/guardian’s name(s) is/are other than that on these documents, evidence of legal name change is required.

4. Provide Parental Identification (for minors under age 14)

Each parent or guardian must submit one of the following:

  • Valid Drivers License
  • Valid Official U.S. Military ID
  • Valid U.S. Gov’t ID
  • Valid U.S. or Foreign Passport with recognizable photo
  • Naturalization/Citizenship Certificate from INS with recognizable photo
  • Alien Resident Card from INS

NOTE: Your Social Security Card does not prove your identity.

If none of these are available, you will need:

1. Some form of signature ID, and

(Ex: Social Security card, credit card, bank card or library card)

2. A person who can vouch for you. He/she must

  • Have known you for at least 2 years,
  • Be a U.S. citizen or permanent resident, AND
  • Have valid ID.
  • Fill out Form DSP-71, Affidavit of Identifying Witness, in front of the passport agent.

5. Present Parental Application Permission Documentation (for minors under age 14)

  • Both parents must appear together and sign

- OR -

2. One parents appears, signs, AND submits second parent’s written statement of consent (including the child's name and date of birth) authorizing passport issuance for the child

- OR -

3. One parents appears, signs, and submits primary evidence of sole authority to apply (such as one of the following):

  • Child’s certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent
  • Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only applying parent
  • Court order granting sole custody to the applying parent (unless child’s travel is restricted by that order)
  • Adoption decree (if applying parents is sole adopting parent)
  • Court order specifically permitting applying parent’s or guardian’s travel with the child
  • Judicial declaration of incompetence of non-applying parent
  • Death certificate of non-applying parent

If none of the above documentation is available, the applying parent/guardian should submit a signed statement (including the child's name and date of birth) explaining why the non-applying parent/guardian’s consent cannot be obtained.

NOTE: A third-party in loco parentis applying on behalf of a minor under the age of 14 must submit a notarized written statement or affidavit from both parents or guardians authorizing a third-party to apply for a passport. When the statement or affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.

8. Provide a Social Security Number

If you do not provide your Social Security Number, the Internal Revenue Service may impose a $500 penalty. If you have any questions please call your nearest IRS Office.

My child who is under 16 needs a passport. How do I get one?

If applying for a child age 13 and under: Both parents or legal guardians must apply with the child.

If applying for a child age 14 to 16: The child MUST appear in person with only one parent.

My child is too young to sign his/her own passport. How do I sign my child’s passport?

In the space provided for the signature, the mother or father must print the child’s name and sign his or her own name. Then, in parenthesis by the parent’s name, write the word (mother) or (father) so we know who signed for the child.

What should I do if my baby is born abroad?

The birth of a child abroad to a U.S. citizen parent(s) should be reported as soon as possible to the nearest U.S. embassy or consulate to establish an official record of the child’s claim to U.S. citizenship at birth. The official record is a Consular Report of Birth of a Citizen of the United States of America, or Form FS-240. This document, know as the Consular Report of Birth Abroad, is a basic United States citizenship document. An original FS-240 is furnished to the parents at the time the registration is approved. (Click here for more information.) A Consular Report of Birth can be prepared only at a U.S. embassy or consulate. It cannot be prepared if the child has been brought back into the United States, or, if the person is 16 years of age or older at the time the application is made.

I was recently married/divorced. How do I change my name on my passport?

You will need to complete Form DS-19, Passport Amendment/Validation Application and send it along with certified documentation of your name change and your current, valid passport to the nearest passport agency. The agencies are listed on the back of the form. Your documentation will be returned to you with your amended passport. There is no fee for this service unless you require expedited service. What Else Should I Know About Passports?

    • All persons, including newborn infants, are required to obtain passports in their own name.
    • If you need to get a valid passport amended due to a name change, use   Form DS-19. (See May I Apply for a Passport by Mail? for the documentation required.)
    • Before traveling abroad, make two copies of the identification page -- one to leave with a friend or relative at home in case of an emergency, and one to keep with you in the event that your passport is lost or stolen while abroad. This will make it easier to get a new passport, should it be necessary. It is also a good idea to carry two extra passport size photos with you.
    • If you run out of pages before your passport expires: Submit   Form DS-19, Passport Amendment/Validation Application, along with your passport to the address listed on the back of the form. Under Other Action Requested, A¢€ write in Add extra pages. Your passport will be returned to you with the extra pages inserted. There is no fee for this service unless you require expedited service. (See How do I get my passport in a hurry?) Also, if you plan to travel extensively, at the time you apply, you may request, at no extra cost, a 48-page passport.
    • Some countries require that your passport be valid at least 6 months beyond the dates of your trip. Check with the nearest embassy or consulate of the countries you plan to visit to find out their entry requirements.

In addition to foreign entry requirements, U.S. law must be considered. With certain exceptions, it is against U.S. law for U.S. citizens to enter or leave the country without a valid U.S. passport. Generally for tourists, the exceptions refer to direct travel within U.S. territories or between North, South, or Central America (except Cuba).
