There are two options for obtaining copies of your passport records:

1. Passport Records for Issuances Prior to 1925
   
   Passport records for issuances prior to 1925 are in the custody of the National Archives. You may write to them at the following address:

2. Passport Records for Issuances 1925 - Present
   Passport Services maintains United States passport information on individuals for the period from 1925 to the present. These records normally consist of applications for U.S. passports and supporting evidence of U.S citizenship, and are protected by the Privacy Act. Passport records do not include evidence of travel such as entrance/exit stamps, visas, residence permits, etc., since this information is entered into the passport book after issuance.
   
   The Privacy Act allows you to obtain copies of records in your own name. To do so, please submit: a typed or clearly printed notarized request, providing your full name, date and place of birth, current address and the reason for the request. Give the estimated date of issuance and any passport information that will enable us to conduct a full search. If you would like an authenticated copy of your own passport records, please submit a check or money order for $20.00 (made payable to the Department of State) to the:
