Obtaining a Passport

The City of Glendale is one of 9,000 acceptance facilities across the nation that accepts passport applications. The Department of State completes the passport issuance at one of 13 regional agencies and mails the passport to your home. All passport applicants, regardless of age, must be present when applying for a U.S. passport book and/or passport card. Children ages 15 and under must be accompanied by both parents. Passport renewals must be mailed to the U.S. Department of State by the applicant.  For complete information on terms and conditions, please visit www.travel.state.gov or call the Department of State information hotline at 1-877-487-2778.

The City Clerk’s passport office is located in the Glendale City Hall building at
5850 W. Glendale Avenue, Suite SB-12, Glendale, AZ 85301. Our telephone number is (623) 930-3260.

Hours

8 a.m. to 3 p.m. every Wednesday, except for legal holidays. We will also be closed December 24, 2008.

Forms and Applications

All forms necessary for passport book and passport card applications (including standard service, lost or stolen passports, renewals by mail, re-application, corrections, or adding Visa pages) can be downloaded at the Department of State webpage www.travel.state.gov

Applying for a Passport Renewal

The City of Glendale does not accept renewal applications. For instructions on how to renew a passport, please visit www.travel.state.gov

What to bring

  • Proof of Citizenship: an original or certified birth certificate with a raised or printed seal from a state vital records office, a Certificate of Naturalization or Citizenship, or an expired U.S. passport. For minors ages 15 and under, a certified U.S. birth certificate is required along with any previously issued passport that is still current.

  • Photo Identification: a driver’s license, state issued ID, or military ID. For a minor under the age of 18, parental ID will be required.

  • Two 2” x 2” passport photos, which can be obtained at any Walgreens, CVS, Kinkos, Costco, or Motophoto. We do not take passport photos at this location.

  • Two personal checks, cashier’s checks, or money orders. No credit card, debit card, or cash payments will be accepted. Please make checks payable to:

Express Mail - Payment for overnight return to the passport applicant by the U.S. Department of State ($16.25) is included in the passport fee payable to the U.S. Department of State.
