All of these documents require the Adobe Acrobat Reader, which can be downloaded for free at www.adobe.com.

Specific event application(s) - Applications must be filled out for each event and must be accompanied by three (3) photographs of the vendor’s work and one (1) photograph of the booth set up. Food vendors are required to submit menus in addition to the photographs. The submission of an application and photographs does not guarantee entry into our festivals. Photographs will not be returned.
Use the following definitions to determine which application fits your business.
- Amusement Vendors: Rides or activities, multiple guest participation based activities
- Commercial I Vendors (CVI): Mass produced product or sales items, company has more than 15 employees
- Commercial II Vendors (CVII): Mass produced product, home based companies, companies with under 14 employees
- Craft Vendor: Majority of products are hand crafted and not resold
- Interactive Vendors: Individual service to patrons, one-on-one interaction (i.e. face painter or henna tattoos)
- Food Vendor: Sells food products that are made for immediate consumption,
including food prepared on or off the event site.
- Glow Novelty Vendors: Selling any glow products, including but not limited to flashing, blinking, neon, glow sticks, glow necklaces, and LED items.
- Non-profit Vendors: 501-C3 status with proper documentation. Non profits are limited and can only sell or pass out information pertaining to the group they are representing. No food sales, prepared and cooked, on site are permitted under this category.
- Non-profit Vendors (Non 501-c3): Non-profit organizations that are non 501-c3 eligible. (i.e. schools, hospitals, libraries, churches, and government units). VENDOR GENERAL POLICIES AND REGULATIONS

BACKGROUND CHECK
The City of Glendale Office of Special Events, through an outside agency, shall conduct a background check on approved vendors participating in any of Glendale’s Festivals, including, July 4th Celebration, Fiesta Glendale, Glendale Glitters Spectacular Weekend, December Holiday Weekends, Glendale Glitter & Glow Block Party, Glendale Chocolate Affaire, the city of Glendale’s Gibson Jazz and Blues Festival. Only one background check is required annually per vendor (July – June). No vendor will be allowed to participate in any festivals until background check is completed, cleared, and approved.
TAX AND LICENSE
All vendors must obtain a business license with the City and State to participate in the City of Glendale’s festivals. Your license must be obtained prior to the event during regular business hours at the Tax and License office. You may obtain a Glendale sales license or occasional sales permit (depending on your gross sales) by contacting the Tax & License Department at 623-930-2233 or in person at 5850 W Glendale Ave 1st floor.
HEALTH DEPARTMENT
All vendors with food items must be pre-approved by the Maricopa County Health Department. Health permits must be displayed in your booth throughout the festival. All pertinent laws, ordinances and regulations pertaining to health and public safety shall be strictly observed. INSURANCE
As an approved vendor you will need to provide the city with a valid Certificate of Insurance (COI). The COI should list the city of Glendale as an additional insured for one million dollars per occurrence naming the specific event that you have been accepted for.

FIRE INFORMATION
If you are using any propane or open flame, please read fire code information. A fire extinguisher, minimum classification 2A10BC, shall be placed in all cooking tents, where deep fat frying occurs an additional Class K fire extinguisher is required. All vendors cooking with flames or flammable materials must use a flame retardant canopy.

VENDOR RULE INFRACTIONS
In an effort to ensure the most professional presentation from our vendors and the best guest experience for the City of Glendale festivals, the Office of Special Events will be monitoring certain aspects of vendor set up, tear down, and presentation. Vendors will now be notified when the following infractions occur. Vendors will be given one warning and on the second violation will no longer be able to participate in City of Glendale festivals.
