The Request for Service form is used by the City Manager's office to document and report concerns, needs and requests from citizens. If you live, work, or are visiting in Glendale and have an issue about which you would like to contact city government, please fill out the following form completely and send it to us via the Internet. We will be glad to assist you or forward your request to the appropriate department. Once we receive your request, we may contact you by telephone to clarify and follow-up on your concerns.

The RFS system is for use by existing Glendale citizens, businesses and visitors in requesting service from the city. It is not designed for vendor registration or establishing sales contacts.
