About the Glendale Fire Department Health Center
The Glendale Fire Department Health Center, along with partners Daisy Mountain Fire District (DMFD), Strength Training, Inc. (STI) and Scottsdale Health Care (SHC) staff, are here to take care of your department’s pre-hire, annual physicals and occupational medical needs. The Glendale Health Center has been in operation since February 2009 and is in the Glendale Regional Public Safety Training Center (11550 W. Glendale Ave.). We currently serve most West Valley fire departments and the Glendale Police Department SWAT/EOD/Clan Lab personnel and the Peoria Police Department SWAT personnel.
What We Do
The Glendale Health Center performs physicals for sworn, civilian and retired personnel. The exams meet AZPOST, NFPA and OSHA requirements and have detected early medical problems. The Health Center also promotes wellness in the areas of substance abuse, diet, exercise and mental health. The fire physicals consist of the following: health survey, urinalysis, blood analysis, hearing and vision testing, pulmonary function, stress test, chest x-ray (done every 3 years), body fat measurement, functional movement screening, physician examination and consultation. Police physicals currently do not include functional movement screening but can for an additional fee. Additional fees for follow-up visits may also apply. In addition to physical examinations, the Glendale Health Center also performs pre-employment drug screening, TB testing, immunizations, hearing tests and other ancillary services.
Hours and Info
Effective 1/3/11 - The Glendale Health Center is open Monday through Friday, 7:30 a.m. to 5 p.m. however the the Health Center will be closed during city holidays. Along with conducting annual physical examinations, we can
manage work-related injuries, work re-entry exams and fit-for-duty exams. For occupational injury needs on a city holiday, you can reference Scottsdale Healthcare locations (click for locations).

As an occupational medical provider, the goal of the Glendale Health Center is to work with your agencies risk management division to provide optimum medical care utilizing our SHC physicians/staff and STI trainers/therapists.
