The most common reasons for changing a name on an account are: marriage, divorce, death and change of occupancy. To assume the responsibility and liability on an account in lieu of establishing a new account, please complete the following on-line form at www.glendaleaz.com under Billing and forms tab or you will need to contact Customer Relations to make the change. In just a few minutes we will be able to process your request. If a deposit is involved it will be necessary for the party surrendering the deposit to complete the name change form and get it notarized, or that person can come in and show ID and sign the form in front of Billing Services Specialist. If the other party on the account will not complete the name change form, a turn-off order needs to be completed for that account and a new account established. The deposit on the account will apply to the outstanding bill and any remaining will be refunded to the parties whose name is on the account.
Department personnel must have access to your utility meters at all times to change service or take readings.
